Affinity Health Plan, formerly known as The Bronx Health Plan, is an independent, non-profit managed health care company serving the needs of low-income individuals living in the New York Metropolitan Area. Affinity was founded in 1986 as the first health plan licensed in New York State designed to serve public sector programs. Affinity health insurance plans include Medicare Advantage, Medicaid managed care, Child Health Plus, Family Health Plus, Family Planning Benefit, and UniCare programs.
To qualify for Affinity Medicare Advantage programs, you must live in one of the five boroughs of New York City and be eligible for Medicare and government payment for some of your Medicare cost-sharing. Affinity Medicaid managed care is available to Medicaid beneficiaries who are eligible through Temporary Assistance to Needy Families (TANF), Transitional Medical Assistance (Home Relief), or Supplemental Security Income (SSI) programs.
Children under the age of 19, who are not eligible for Medicaid or Medicare, can take advantage of the Affinity Child Health Plus program. The Family Health Plus program is available to low income, uninsured adults who are ineligible for Medicare or Medicaid. Affinitys Family Planning Benefit program is offered in certain service areas to provide family planning coverage to individuals who are enrolled in Child Health Plus and Family Health Plus programs and do not receive these benefits. The UniCare program serves uninsured individuals and families but is closed to new enrollment.
